Before submitting this form, please review the US Solar Mounts Corp Limited Product Warranty. Warranty coverage is subject to the terms, conditions, exclusions, limitations, and claim procedures stated in that warranty.
Submission of this Warranty Claim Form does not constitute approval of warranty coverage, repair, replacement, refund, credit, or return authorization.
Warranty eligibility will be determined by USSM after review of the information provided, supporting documentation, photographs, troubleshooting results, service history, installation information, operating conditions, and any products or components requested by USSM.
Please complete all applicable fields as accurately and completely as possible. Incomplete, inaccurate, misleading, or missing information may delay review of your claim or may prevent USSM from determining warranty eligibility.
Do not return any product, component, or part to USSM unless a valid Return Material Authorization (RMA) has been issued by USSM.
Products or components returned without an authorized RMA may be refused, returned to sender, or held pending receipt of required information.
