Return Material Authorization Request
Please review the following information before submitting the RMA Request Form.
Use this form to request a Return Material Authorization (RMA) before sending any product, component, or part to US Solar Mounts Corp.
No product or component may be returned to USSM unless a valid RMA number has been issued by USSM.
Submission of this RMA Request Form does not constitute approval of warranty coverage, return authorization, repair, replacement, refund, or credit.
Products or components returned without an authorized RMA may be refused, returned to sender, or held pending receipt of required information. Returned items must be properly packaged, reasonably clean, dry, and free of hazardous substances or biological contamination.
Please complete all applicable fields and attach any requested photographs, proof of purchase, service records, troubleshooting information, or other documentation needed for USSM to evaluate the return request.
